hello dear,
you seem to have an anal fissure with a sentinal pile. this is due to a tear in the anal area which heals by resulting in narrowing of anal canal and thus tears again. The narrowed anal opening needs to be brought back to normal size.

for the sentinal pile, there is no need to remove unless it is complicated by infection, twisting, bleeding or poor hygiene issues.
