Better go for Voice therapy first from a good speech therapist

If your puberphonia is persisting even at 20 years, you definitely need a treatment. You have to consult an ENT Specialist who treats Voice Disorders & most probably he will advice a course of Speech Therapy (at least for 6 weeks) initially. If even after speech therapy you do not feel improved then only we consider surgery. So do not worry till then.
