Hello there .. Kindly specify when you say same braces. By braces do you mean fixed braces with wires & elastics around them or do you actually mean Retainer appliance which is given after the completion of the fixed therapy. Dental problems after prolonged orthodontics can vary from periodontal(gum) problems to dental Decay(caries) depending on the oral hygiene practice of your friend. One needs to keep a meticulous oral hygiene during braces. Regular flossing , minimum two times brushing & use of fluoridated dentrifice is the key for for keeping a good oral hygeine.

20 years is a long span of time. Are you sure it is "braces" and not retention wire? In some cases a retention wire is applied on inner surfaces (lingual side) of teeth but if these are braces, they must be removed now. After that you must also undergo scaling and polishing of enamel. And at last yes it is a cause of concern. It may result in tooth decay, periodontitis, increased chances of low birth baby weigh, halitosis etc.

Hi..... wearing the same Braces for 20 yrs...????.. Is your friend not visiting the Dentist with whom he/she has applied the braces??. Anyways, First thing to verify is , whether, the patient is wearing fixed wires with braces on front teeth surface and brackets on back teeth ??. or are the wires removable one (retainers) , or are the retainers on inner teeth surface (lingual) fixed.ones........If fixed braces on anterior(front) surface.....den remove them ASAP and get the teeth Scaled......n check for cavities
