I am diabetic since 1993 at 37 years of age. In 2008, I have been attacked by CHD with occlusion of LAD artery which has stunted at that time. I am taking the following medicines from that time: 1- Tab. Metformin 500 mg since 1993 as thrice a day between meals. 2- Insulin Levemir ( Long acting) since 2012 3- Insulin Novo Rapid since 2012 After CHD the following are included: 1- Tab. As car 75 mg at breakfast. 2- Tab. Bisoprolol Hemifuramate 2.5 mg at breakfast. 3- Tab. Clopidogeral 75 mg at night. 4- Tab. Ramipril 5.0 mg at night. 5- Tab. Rusuvastatine 10-20 mg T night. Tab. Vit. B12 1+1+1 off & on. Now my; @ S.Cholesterol 146 mg/dl @ S. Triglyceride 120 mg/dl @ HDL 40 mg/dl @ LDL 167 mg/dl @ SGPT/ALT 110 u/l @ S. Creatinine 1.8 mg/dl @ Hb 12 gm/dl @ ESR 70 mm/1st. Hour @ TLC/WBCs 7100 /Cu.mm @ RBCs 2.97 Mill/Cu.mm @ Platelets 124000 /Cu.mm @ PCV 25.6 % @ Neutrophils 75% @ Lymphocyte 20% @ Monocytes 3% @ Eosinophils 2% I have no any symptom except minor muscle pain. I am taking sugar free diet only vegetables & chicken. I am taking abou 10-glass water since 2008. Now what should I do to lower SGPT, to lower LDL, to increase Platelets etc Dr. Ijaz Sandhu

You are taking 2 blood thiners (antiplatelet medications) - As car and clopidogrel. After stunting you had to take both of them together for just a year, after 1 year just one of them is left in treatment plan-As car or clopidogrel. So, take just As car (75 mg after meal, x1 times a day) and cancel clopidogrel! It seems that your muscle pain and increased ALT, AST (SGPT) is the result of rozuvastatin therapy, you should decrease the dose-take 5-10 mg a day and after 3 months check your liver enzymes-ALT, AST (SGPT). If they are x3 times high above normal, statins should be stopped for some time. For now it's not indicated to stop rozuvastatin, because your LDL is high, in addition you have ishcemic heart diseases and should take statins to the end of life, but dose needs to be adjusted according to liver enzymes. Also, you should have low lipid diet! You should visit your cadriologist regularly!

My father is 78 years old and has been suffering from constipation for the last 2 years. He has also lost a lot of weight around 10 kgs in this time span. In the last couple of months he has been complaining that his abdominal muscles have torn due to bowel straining and now he says there is loss of muscle in his stomach area. Which is why when he lies down everything is fine but when he sits up and walks around he feels like he is carrying a weight in his stomach. After intake of food it becomes worse and he also gets bloating. We have done all possible tests - colonoscopy, CT scan, ultrasound sometime last year and everything was clear. The doctors gave a verdict of idiopathic constipation and left it at that. His blood work is fine, haemoglobin levels, cholestrol etc. He still suffers from constipation. Sir i am suffering from knee sweeling in my rt knee for 6 month,two times i hv removed water(aspiration) from it bt never found relief.some time it decrease and some time increase.all tests x rays,crp,esr are normal not yet i found any relief.4 years ago same problem were in my lft knee and dr. Removed 2 pieces(tissue) from lft knee which were found normal(non specific),till one month after surgery i couldn't bent knee even a little bit,i took physiotherapy by wax and till 9 months i could bent it onle 80% after 1.5 or 2 year i could bent it properly 100 bt with pain,still there is pain when i bent it proper,and also swelling in rt knee.Dr.'s Of hospital in which i get surgery 4 yrs ago said that same surgery will pass in rt knee,bt i am afraid because still there is pain in my lft knee,if i pass again surgery in rt knee i will face same problem of pain in my rt knee,please advice me sir if there another way to get treated without surgery like med. About 5 years ago I noticed a dramatic reduction of the muscle mass (Muscle atrophy) in my right calf muscle, like 75% to 80% reduction compared to my Left calf Muscle (possible the gastrocnemius), I also noticed that my Achilles is also very loose, when in a relaxed state, compared to the left leg. What could be the possible problems? There is no pain (other than uneasiness like restless leg syndrome) associated with this problem. I do have a lower back problem (damage) that was done 30 years ago to the L4/L5 herniated disk. Now I have noticed that other muscles are starting to harden and reduce like the one did 5 years ago. I went on a 15 mile hike in the mountains this past summer and I am very active during the day. However I do not regularly exercise as I should. Thoughts?

My wife had a HSG Test done w/ x-rays 4 days ago. Last night she was complaining about a sharp pain in the lower left quadrant of her lower abdomen. She said it was very painful and was not going away or becoming less intense. By the time for today came, she was complaining about the pain, it being 5/5 at times. She described this pain as a sharp stabbing constant pain. However, now the pain is radiating around the lower left of her abdomen to her lower left back. The area is very tender and the pain spikes even when slightly pushed on in both the areas, abdomen and lower back. Is this a common side-effect of having the HSG Test? Especially if both of her tubes are blocked and she has been diagnosed with Endometriosis. Please help me figure this out so I know whether or not she needs to be seen at a hospital.

GERD. It has been on and off since 2008. The condition worsened last month in Jan 2013 and I could hardly swallow any solid food. It just gets stuck in the throat and feel like I am choking till I push the food down with water. Liquids and semi solid foods are ok. The doctors did Nasal Endoscopy, Normal Gastro Endoscopy, Esopheagal Manometry, Barium Swallow X-Ray and all tests came up normal. I was told the swallowing problem is primarily related to GERD and I was advised Cintapro 1 mg tablet for 1 month. So far as I take the tablet, I am able to take in selected solid foods fine, but not all foods. If I do not take this tablet for one day, the problem recurrs back. Question 1: How long will this motility disorder be related to GERD ? Question 2: Are there any side effects of taking this Cintapro 1 mg to help in motility on a long term basis (Say if I need to take this for some more time) Questionj 3: Are there any other tests that will show the proper reason for this motility disorder of the esophaegus (Apart from Esopheagal Manometry, Gastro Endoscopy, Barium Swallow X-Ray) etc.

Dear Doctor, Please help me to control my diabetes. I am a dibetic patient about 17 years and under insulin for the past 5 years.Before five years I underwent CABG surgery. My need for consultation is for Diabetes. Now i take Metformin 500 SR/Diapride 2mg and 30/70 fast acting Insulin 18 in the morning and night time I take diapride 2mg and Insulin 8 points only. My HbA1c is 7.9 and always PP is above 200 to 240 Today fasting is 121 and PP is 234.Please advice.

Everytime I get into my car with AC on after some distance of journey I get severe dry lips. i went for my complete body checkup.every thing was fine except my liver,the sgot and sgpt,billirubin was fine but my ggtp was high about 121.even my sugar level H1Blc was 5.5 with very low medication . i am taking metformin 250 gms two times daily after meals.another problem i am facing is lack of sleep for which i am taking 1mg of alprax every day.but still i am not able to sleep.I have to mention that i am a successful businessmen with stressful life.please suggest something
